Application

This memorial was established to honour those drivers who have been killed whilst performing their duties behind the wheel.

We also have a separate memorial wall for those significant contributors to the transport industry, who have died from causes other than a truck road accident.

It is the responsibility of the applicant to inform other family members and friends of this inscription, as only the contact person listed on this form will receive correspondence from the Australian Truck Drivers’ Memorial Board.

There is no charge for inscription if your request is accepted, however as we are a non-profit group, you are more than welcome to send a donation of any amount to help the Memorial Wall continue to be supported and maintained. It would be greatly appreciated.
